Theresa Jack

Transition Specialist

Theresa Jack has been with Smooth Transitions since 2018. Theresa grew up in the Finger Lakes Region of New York State. She has over 25 years’ experience as a Senior Advocate, in various capacities. In her role with Smooth Transitions, Theresa greatest joy is providing her clients with the services and goods they need. 

Theresa graduated from James Madison University in Virginia with honors. She currently serves on the board of directors for Smithfield Rescue Mission whose mission is to break the cycle of poverty and homelessness. She currently lives in a forest with her husband of 35 years a few four-legged family members.
